Activity Agreement

1. Acknowledgment and Assumption of Risk

I, the undersigned parent, guardian or adult athlete (over 18), understand that my child (for the purpose of this agreement "child" will refer to the participant in the activity) will be participating in activities at Star Spirit, including cheerleading, dance, tumbling, gymnastics, aerial arts, and related activities. I acknowledge that these activities involve inherent risks, including sprains, strains, bruises, fractures, concussions, and, in rare cases, serious injury or even death. While Star Spirit provides supervision, safety measures, and experienced instruction, it is important to understand that these risks cannot be entirely eliminated.

2. Voluntary Participation & Medical Clearance

My child’s participation is voluntary, and I confirm they are in good health with no medical conditions that would make participation dangerous. If necessary, I will consult a doctor and provide clearance before participation. I agree to notify Star Spirit of any changes to my child’s health status that may affect their ability to safely participate.

3. Liability & Personal Responsibility

I understand that Star Spirit is only responsible for injury or damage caused by their negligence. I accept that I am responsible for any medical costs resulting from participation. I agree to follow all safety rules and guidelines to reduce the risk of injury, and I will ensure my child does the same.

4. Behaviour Expectations

At Star Spirit, we encourage a positive and respectful environment:

For Athletes: Follow instructions, be respectful to others, and focus on training.
For Parents/Guardians: Show respect for coaches, staff, and other families. Coaching from the sidelines is not allowed.
Any negative talk or behaviour about other members—whether online, in person, or in any other form—will not be tolerated and may lead to disciplinary action, including removal from the program.

5. Competition & Social Media Conduct

Respectful behaviour is expected at all times, including competitions and online. Inappropriate posts about Star Spirit, or disruptive behaviour towards athletes, staff, or event staff, will result in consequences. Parents and athletes are responsible for maintaining a positive image of Star Spirit, both in-person and online. Any action that brings Star Spirit’s reputation into disrepute may lead to removal from the program.

6. Training at Other Facilities

I understand that training at other facilities can lead to increased injury risk, inconsistent coaching, and conflicting training techniques. My child will not participate in cheerleading, gymnastics, dance, or aerial activities at any other facility without first discussing it with and obtaining approval from Star Spirit staff. Training elsewhere may put my child’s place on their team or teams at risk.

7. Media Release

I consent to Star Spirit taking and using photographs and videos of my child for promotional, health & safety, and other purposes. If I prefer my child not to appear in social media, I will notify Star Spirit in writing before participation.

8. Acknowledgment of Contract

I understand that this is a legally binding contract and agree to its terms. I have read and understood the contents of this agreement.
