1. Acknowledgment and Assumption of Risk
I, the undersigned parent/ guardian, understand that my child is participating in cheerleading, dance, and/ or aerial arts which involves inherent risks. These activities can result in injuries such as sprains, strains, cuts, bruises, fractures, and in rare cases, more serious injuries or even death. I acknowledge that while safety measures and instruction are provided, these risks cannot be entirely eliminated.
2. Medical Clearance
I confirm that my child is in good health and has no known medical conditions that would prevent safe participation in these activities. If unsure, I have consulted a physician who has approved their participation.
3. Personal Responsibility
I agree to speak to my child and make it clear that they are to follow all instructions and safety guidelines provided by the instructors and staff. I understand that failure to do so may result in increased risk of injury. I have told them that they need to inform staff of any injuries or conditions that arise during participation.
4. Waiver and Release
In consideration of being allowed to participate in the cheerleading, dance, and aerial arts program, I hereby release, waive, and discharge Star Spirit Athletics ltd (Star Spirit Cheer and Dance), its officers, instructors, employees, and agents from any and all liability for any injury, loss, or damage to myself or my property, except where such injury, loss, or damage is caused by the negligence of Star Spirit Athletics Ltd or its staff.
5. Emergency Medical Treatment
I authorise Star Spirit Athletics Ltd to seek emergency medical treatment in case of an accident or injury.
6. Photography and Media Release
I consent to Star Spirit Athletics Ltd using photos and videos taken during classes or events for promotional purposes and internal reporting of skills. I understand that I will not be compensated for such use. If my child cannot be posted on social media I will make it clear via email to hello@starspirit.co.uk what can and cant be used.
