For the document below, “child” refers to the named child provided above.

1. Acknowledgment and Assumption of Risk

As the parent or legal guardian, I understand that my child is taking part in cheerleading, dance, and/or aerial arts — activities that, while fun and empowering, do carry some inherent risk. I acknowledge that despite all safety measures in place, injuries such as sprains, strains, cuts, bruises, or more rarely, fractures or serious injury, can occur. I accept these risks and trust that all instruction and supervision provided by Star Spirit Cheer & Dance will prioritise my child’s safety.

2. Medical Clearance

I confirm that my child is in good health and able to take part in physical activity. If there have been any health concerns, I have consulted a medical professional who has confirmed that it is safe for my child to participate. Any condition noted above does not limit their participation in class.

3. Personal Responsibility

I agree to speak to my child about the importance of listening carefully to their coaches, following instructions, and respecting all safety guidelines. I understand that doing so helps to keep them and others safe. I’ve also made sure they know to speak to a staff member if they ever feel unwell, sore, or experience any discomfort during or after class.

4. Waiver and Release

In return for my child being allowed to participate in Star Spirit Cheer & Dance programmes, I agree to release Star Spirit Athletics Ltd (trading as Star Spirit Cheer & Dance), including its staff, instructors, and representatives, from liability for any injury, loss, or damage, except where such injury, loss, or damage results from negligence on the part of Star Spirit Athletics Ltd or its team.

5. Emergency Medical Treatment

In the event of an emergency, I authorise Star Spirit Athletics Ltd to seek appropriate medical care for my child.

6. Photography and Media Consent

I give permission for my child to be included in photographs or videos taken during classes and events, which may be used for promotional purposes or internal skill tracking. I understand no compensation will be provided for this usage. If I prefer my child not to be featured in promotional material or social media, I will clearly communicate this by emailing hello@starspirit.co.uk with details of what can and cannot be used.

Monthly Subscription

By enrolling on a Monthly Subscription, you agree to an initial and recurring monthly tuition fee, charged at the agreed subscription rate. You accept responsibility for all recurring charges until a cancellation request is submitted in line with the cancellation policy.
Tuition payments are processed automatically on the 1st of each month through our secure AutoPay system.
Monthly fees will continue to be processed at the subscription rate unless a cancellation request has been submitted and confirmed.

Automatic Renewal

Subscriptions automatically renew each month without the need for further authorisation.
Payment will continue to be collected on the 1st of each month until a cancellation request is submitted.

Cancellation Policy

A cancellation form must be completed and submitted before the 15th of the month to stop the following month’s payment from being processed. Cancellation form available by emailing hello@starspirit.co.uk.
Any payments already collected prior to the cancellation taking effect are non-refundable.
Training will remain available until the end of the paid subscription month.
